Heather Nicole Moseley Memorial Scholarship

The Heather Nicole Moseley Memorial Scholarship is provided to graduating or currently enrolled students of Occupational or Physical Therapy. The scholarship was established in 2008 in memory of Suzanne and Homer Brantley's granddaughter, Heather. The $500 fund is renewable for up to eight semesters.

    The applicant must be a legal permanent resident (or U.S. citizen).
  • Citizenship:
    The applicant must be a U.S. Citizen (or a legal permanent resident).
  • College:
    Applicant must attend Amarillo College, West Texas A&M University, Texas Tech University or the University of Texas.
  • Major/Career:
    The awardee must major in Occupational or Physical Therapy (the Pre-Occupational and Pre-Physical Therapy programs, as well as the Occupational and Physical Therapy Assistant programs also qualify).

  • Recommendation Letters:
    Two (2) letters of recommendation - one letter should be from a teacher you had in high school and the second letter should be from a pastor, employer, coach, mentor, etc. Letters written by family members are not permitted.
  • Other Materials:
    Applicant must submit their proof of U.S. Citizenship (photocopy of U.S. birth certificate) or legal permanent residency status, ACT and/or SAT scores (if scores appear on the back of your high school transcript, no additional documents are necessary), page one (1) of current year IRS tax Form 1040 for household (if current year is not available, previous year will be accepted).
