The Foundation for Appalachian Ohio

The OACAC Charles L. Warren Memorial Heritage Scholarship is provided to a high school senior of an OACAC member school. The program grants a one-time $1,000 scholarship for applicants who demonstrate and exhibits a commitment to cultural and intellectual diversity.

    Applicant must submit their Personal Statement. Please submit a typewritten response to each of the following questions.1. Please describe the environment that you come from for example, your family, community, or school and how this environment has influenced what you value most about your heritage. (250-word limit)2. How have you currently contributed to the intellectual and cultural diversity in your high school or community, and how do you plan to continue your commitment at your college or university of choice? (250-word limit)3. How will this scholarship make a difference to you? (100-word limit)4. Please list any colleges and or universities to which you are applying or plan to apply (Please keep in mind that you must attend an Ohio College or University to be considered for this scholarship).
