    250-word essay describing your family, church involvement (including where you are a member and for how long), and future plans

    Two letters of recommendation: one from a church leader and one from a teacher.

    Have proof of acceptance into a college or university with the intent of completing an associates or bachelors degree;Proof of GED, if applicable; List of church activities in which you have participated, including number of years participated for each activity;
